I. A. Safe Motherhood

a. Pre-natal Check-up

  1. Client will proceed to the FP room for admission
  2. HBMR card is being issued and properly filled-up
  3. Buntis Class will be conducted
  4. Individual check-up of clients.
  5. Giving of Tetanus Toxoid infection; ferrous sulfate; Multivitamins & abdominal palpation
  6. Instructions when to return for next visit
  7. Referral if needed

  1. Midwives are instructed to attend delivery call to client in labor

  5. Observation and monitoring of vital signs after delivery

I.B. Family Planning

  1. Client will proceed to the family planning room and give personal data to midwife for recording
  2. Midwife/nurse present all FP method available including its advantages & disadvantages and let the client select what is suited for her
  3. Midwife/nurse do physical examination before giving the method
  4. Health teaching/advice when to come back for re-supply

II. Child Care

a. Immunization

  1. Client (mother of the baby or guardian) will proceed to immunization room 2.
  2. Client shall be issued with growth monitoring card EPI record & properly filled-up by the midwife
  3. Client is instructed to take a sit and wait for the lecture
  4. Lecture proper
  5. After the lecture, client is advised to wait for their babies to be called for the immunization proper
  6. Advise client when to come back
  7. Refer if needed
  8. Follow-up at home is made by the midwife for defaulter

III. Prevention and Control of Infectious Diseases

1. TB-DOTS

a. Case finding

  1. Client will register at the admission table for proper recording
  2. Vital signs & history taking by midwife or OD
  3. Client shall be referred to the nurse or MHO for assessment
  4. Refer to med. tech. for AFB
  5. Health teaching
  6. Advise when to come back

  5. Health teaching is given before giving the initial supervised dose of SCC
  6. Advise when to come back for re- supply

1. Requesting Certificate of Pre-marriage Counselling Seminar as pre-requisite for applying marriage license

  1. Client approach the employee-in charge (EIC) & request for the needed documents

  3. Couples fill-up the Marriage Expectations Inventory form
  4. Records information/data gathered from the couple
  5. EIC give instructions regarding schedule & venue of the seminar
  6. EIC issue charge slip & instruct couples to pay to the Treasury Office PMC fee
