1. Secure and Fill-up Assessment Form

Fill up and submit Application Assessment Form along with all requirements.

Licensing Officer computes taxes fees and other charges.

3. Approval of Assessment

Officer (EEDMO) reviews and recommends aooroval of the assessment ana affixes his signature on assessment form.

Before the final approval of the Municipal Mayor, the applicant must proceed to the Municipal Environment and natural Resources Officer (MENRO) for a briefing on RA 9003. The MENRO affixes his initial at the form.

The Municipal Mayor or authorized signatories signs the assessment form for final approval.

4. Payment of Business Taxes, Fees and other Charges

The applicant proceeds to the Municipal Treasurer's Office for payment of assessed fees. The applicant may choose to pay on an annual, semi-annual and quarten basis. 5.

5. Secure Clearances

Payment of fees for new firms and applicants for renewal for their clearances from the:

  • Municipal Planning and Development Coordinator
  • Municipal Engineer
  • Municipal Health Officer
  • Bureau of Fire Protection
10-20 minutes

Time frame depends on the completeness of the requirements needed for the clearances. Also, for new applicants, actual inspection is conducted before the issuance of Building Permit, Zoning Permit and Fire Inspection Safety Certificate

6. Secure Mayor's Permit and Registration Plate/Sticker

Upon presentation of the official receipt and secured clearances, the licensing officer prints the Mayor's Permit of the applicant

7. Approval of the Mayor's Permit

After the approval of the Mayor's Permit, the applicant goes back to the Licensing Officer to leave the duplicate copy of the permit.
